For your convenience, we accept Visa, MasterCard, and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We recognize the need for a definite understanding between our office and you regarding financial arrangements for surgical care.  Our fees are comparable to the charges made by like specialists in the area.  Although we attempt to advise you of all fees, please be aware that the treatment can change.  

Many insurance companies have fixed allowances or percentages based upon your contract with them, not with our office.  While we accept most insurance plans, we are not in-network with all plans; it is your responsibility to check with your plan regarding specific coverage.  Please understand that it is your ultimate responsibility to pay the deductible, coinsurance and any other balances not paid by your insurance carrier.  Any insurance reimbursement will be credited to your account. 

As a professional courtesy to our patients, we will complete and submit all claims directly to your insurance carrier(s).  However, having insurance is not a substitute for payment, as we cannot guarantee insurance benefits.  Any balance remaining after insurance pays will be billed to you. 

Our office will provide you with a proposed treatment plan for your surgical care.  We will attempt to contact your insurance carrier(s) prior to your surgery to verify your benefits and we will notify you of your estimated benefits as well as out of pocket expense.  Charges for services not covered by your insurance carrier(s) are your responsibility. A portion of your estimated out of pocket balance due may be due at the time of service.  

We submit insurance claims with accurate and valid procedure codes along with additional information requested by insurance companies.

We try to be as accurate as we can when contacting your insurance for benefits upon your request. Your insurance company may tell us certain procedures are or are not covered, but then process them differently.  This sometimes accounts for a difference between the estimated insurance payment and your actual out of pocket expense.  Knowledge of benefits as well as benefit amounts, limitations, exclusions, waiting periods, etc. is entirely YOUR responsibility. We make every effort to collect insurance money that is due to us for services rendered. If insurance is not resolved 60 days after the last claim submission and we have exhausted all our efforts, you will become responsible for your balance regardless the status of the insurance.
